Connection Through Collaboration: Health Impact Ohio Hosts First CVS Learning Collaborative

In May, Health Impact Ohio (HIO) proudly hosted the first CVS Learning Collaborative, bringing together Care Coordination Agencies, Community Health Workers, and partners for a day of learning and shared commitment to improving health outcomes across our communities.
We extend a special thank you to Columbus Metropolitan Housing Authority (CMHA) and CVS Health for their partnership and for providing an outstanding meeting space at the Health Workforce Innovation and Talent Center at Rosewind Estates. Their hospitality and dedication to workforce development created the perfect environment for meaningful conversations and collaboration.
Representatives from Physicians CareConnection, Ebenezer Healthcare Access, Wellness First, St. Vincent Family Services, Holistic Pregnancy & Wellness, Compass at Broad Street, Star House, HIO, CMHA, and CVS Health attended, connected, shared experiences, and learned from one another while strengthening relationships that support the communities we serve.
A highlight of the day was a guided tour of the center, where CMHA and CVS representatives showcased the innovative pharmacy technician training program offered onsite. Participants gained valuable insight into how workforce development initiatives are creating pathways to meaningful careers while addressing healthcare workforce needs in Central Ohio.
The collaborative also provided an opportunity to reflect on the progress of our CVS initiative during the first six months of implementation. Together, our Care Coordination Agencies have:
Served 179 clients
Completed 942 pathways across five active Care Coordination Agencies, with two additional funded agencies currently completing hiring and onboarding
Achieved an average of 2.5 days from referral to initial CHW contact
Maintained an average client engagement period of 183 days within the HUB
These early outcomes demonstrate the effectiveness of coordinated care and the dedication of our CHWs in connecting individuals with the services they need.
One of the most impactful discussions centered on first-trimester pregnancy enrollments. Participants engaged in a thoughtful roundtable conversation about current challenges, successful outreach strategies, and opportunities to be more intentional in identifying and enrolling pregnant individuals earlier in their pregnancies. Early engagement remains a critical factor in improving maternal and infant health outcomes, making this an important area of focus for the collaborative moving forward.

The first CVS Learning Collaborative demonstrated the power of bringing partners together to celebrate progress, identify opportunities, and align around shared goals. Through collaboration, data-driven conversations, and a collective commitment to community health, we are building stronger systems of care that improve lives across Ohio.
