Empowering Central Ohio: Insights from the Pathways HUB Referral Data

The Central Ohio Pathways HUB is dedicated to connecting individuals and families with the resources they need. Our recent look at the referral data from our public form, which served 2025 clients, offers an eye-opening glimpse into the community we serve — including details on health coverage, housing status, demographics, and the local agencies that help us provide care.

Insurance Coverage: A Strong Medicaid Presence

Our data shows that the majority of our HUB clients are covered by Medicaid, highlighting the HUB's role in supporting individuals and families with limited financial resources. This coverage helps them access a wide array of health services, aligning with our mission to break down barriers to care.

Housing Status: Addressing Homelessness Head-On

A notable portion of referrals come from individuals identifying as homeless. This finding underscores the continued importance of housing support within the HUB's network. Housing stability is essential for health and well-being, and the HUB's integrated care approach ensures that homelessness is met with coordinated assistance.

Gender Distribution: Inclusive Engagement Across the Community

While a majority of our clients identify as female, we strive to serve a diverse array of gender identities. This trend aligns with statewide data showing women often act as key advocates for family health and community well-being.

Age Demographics: Supporting Adults in Their Prime

Most clients fall within the 18–65 age group, indicating that the HUB primarily supports working-age adults managing multiple health and social needs. A smaller but meaningful share of clients are youth under 18 or seniors 65 and older, reflecting our commitment to individuals across the lifespan.

Partnering Agencies: Collaboration at the Core

The referral data illustrate the collaborative nature of the HUB model. The Ohio State University is one of our key partners, contributing to multiple referral programs and actively participating as a Care Coordination Agency (CCA) through their Department of Emergency Medicine. This partnership is crucial in helping us connect clients to comprehensive care. The diversity of agencies participating in the HUB shows how partnerships strengthen the HUB's ability to address social determinants of health.
