A trusted individual who lives, works, or worships in the community that they serve. CHWs are employed by community-based organizations, FQHCs, Clinics, Hospitals, and more, where they are experts at connecting their clients with resources and services that address the individual’s health related social needs.

• 18 years old or older (unless in the High School Program)

• Have at least a GED or High School Diploma

• Love engaging with your community

• Working as or interested in working as a CHW or similar job title connecting clients with resources and services in the community

• Able to make a 12-week online class Monday, Tuesday and Thursday from 3-5pm

• Able to attend an in-person graduation (5 hours) in Columbus, Ohio

• Able to work 130 hours and complete a Practicum Placement within six months of graduation

HIO program requirements:

• 12-weeks of online class on Monday, Tuesday, and Thursday from 3-5 pm

• In-person graduation (5 hours) in Columbus, Ohio

• 130 hours of Practicum Placement work within six months of graduation (can be completed at your current place of employment if applicable.)
