Health Impact Ohio continued its Community Health Worker (CHW) Profile Project, interviewing various Central Ohio Pathways HUB CHWs on their experiences and successes.

This month, Health Impact Ohio's Chief of Policy & Partnerships and CEO of the Central Ohio Pathways HUB, Jenelle Hoseus, interviewed Tori Owens and Patrice Howard, who are CHWs at Physicians CareConnection. Tori and Patrice spoke on their shared passion for working with individuals experiencing homelessness and their motivations for becoming CHWs.

Health Impact Ohio Hosts a Joint Conference with Fellow Regional Healthcare Improvement Collaboratives

In continuing the tradition of bringing together multi-stakeholder groups interested in advancing primary care in the state of Ohio that was formerly managed by Ohio Patient-Centered Primary Care Collaborative (OPCPCC), Health Impact Ohio along with our two fellow Regional Health Improvement Collaboratives (RHICs) hosted a hybrid conference accommodating both in-person and virtual attendees. This joint learning opportunity, held by Health Impact Ohio, Better Health Partnership, and The Health Collaborative, aimed to bring primary care stakeholders from across the state together to discuss the future state of health care in Ohio. Former OPCPCC leader Dr. Ted Wymyslo opened the day with welcome remarks.

The Heart Healthy Ohio Initiative (HHOI): Full Speed Ahead with Quality Improvement Efforts

Ohio is in the top quartile of states for high cardiovascular disease (CVD) event rates, combined stroke and heart attacks, highlighting the need for the Heart Healthy Ohio Initiative (HHOI) Quality Improvement Project (QIP). HHIO QIP is funded by the Agency for Healthcare Research and Quality and led by several of the medical schools and the three Regional Health Collaboratives across the state. This QI initiative is partnering with the Ohio Cardiovascular and Diabetes Health Collaborative (Cardi-OH) and the Ohio Academy of Family Physicians and is focused on adapting and testing a Heart Healthy Quality Improvement (QI) process designed to improve cardiovascular health and reduce disparities in CVD. 

Health Impact Ohio Hosts Community Health Worker Day Celebrations!

In celebration of June 6th officially being declared Community Health Workers Day, Health Impact Ohio hosted a day of relaxation for CHWs in the Central Ohio Pathways HUB offering lunch and massages to all CHWs who attended. Health Impact Ohio was honored to be able to show support and appreciation to all CHWs and give a special thank you to all the amazing CHWs in the Central Ohio Pathways HUB who work tirelessly every day as trusted messengers to serve their communities.
