Community Health Worker Profile Project

Health Impact Ohio continued its Community Health Worker (CHW) Profile Project, interviewing various CHWs on their experiences and successes.

This month, Health Impact Ohio would like to showcase a video interview conducted by our HUB Project Manager, Mary Mutegi, and Community Health Worker, Delores Richardson. Delores shares her experience working with clients with diabetes.

Health Impact Ohio Releases 2022 Impact Report

The Central Ohio Pathways HUB Begins Participation in the Diabetes Self-Management Education (DSME) Program

The Central Ohio Pathways HUB has been, since it’s inception, working with the Medicaid Managed Care Plans and other partners to showcase how the HUB model, when used to fidelity, can truly impact the health and well-being of our neighbors.

In an acknowledgment of the Ohio Department of Medicaid’s vested interest in positively impacting the health of all Ohioans through requirements on improvement in member outcomes from the plans, the HUB has consistently offered to participate in potential pilot projects that would allow for additional supports for success.

Health Impact Ohio Visits HUBs Across Ohio

This month, Health Impact Ohio began visiting HUBs across the state of Ohio as part of the Community Health Workers (CHWs) for COVID Response and Resilient Communities grant. We have officially met with six out of the twelve HUBs that are participating in the grant and will continue to visit the remaining HUBs throughout the month of February.

We are thrilled to have the opportunity to meet CHWs from across the state in person that we have worked with since December of 2021 and look forward to training and collaborating together throughout 2023.
