Central Ohio Pathways HUB Surpasses $5 Million in Funding to Local Nonprofits, Strengthening Community Health Efforts

Columbus, OH – February 2025 – Health Impact Ohio (HIO) proudly announces that the Central Ohio Pathways HUB (the HUB) has surpassed a remarkable milestone, distributing over $5 million to local nonprofit organizations since 2019. This funding has been instrumental in supporting Community Health Workers (CHWs) and Care Coordination Agencies in their mission to connect Central Ohio’s most vulnerable populations to essential health and social services.

Since its inception, the HUB has worked alongside Managed Care Organizations (MCOs), local nonprofits, and healthcare providers to improve health outcomes across the region. In 2024 alone, HIO administered $850,100 to HUB Care Coordination Agencies, ensuring sustained efforts in community-based care.

Community Health Workers (CHWs) play a critical role in addressing social determinants of health and improving health outcomes for vulnerable populations. By acting as trusted advocates and navigators, CHWs bridge the gap between individuals and the often-complex health care system. Through innovative models, legislative support, and dedicated training programs, Ohio is paving the way for a more effective and equitable approach to care delivery.

This topic was explored in depth during the Civitas Networks for Health Annual Conference in October 2024, in a session led by Jenelle Hoseus, a Civitas board member and Chief Policy and Partnerships Officer for Health Impact Ohio, and Chief Executive Officer for Central Ohio Pathways HUB, alongside Tanikka Price, Chief Education & Equity Officer for Health Impact Ohio.

Explore Ohio School-Based Health Alliance SBHC 101 Toolkit

The Ohio School-Based Health Alliance (“Ohio Alliance”) recently released the SBHC 101 Toolkit, a comprehensive resource designed to support school districts, healthcare providers, and community partners in exploring and setting up school-based health centers (SBHCs). SBHCs are an evidence-driven model that require school district and healthcare provider partnership to bring access to, at minimum, comprehensive primary care services to students at school.

The SBHC 101 Toolkit is divided into three modules, offering valuable information, resources, and tools to guide partners in: (1) Understanding the SBHC model, including key components and its impacts; (2) Conducting a comprehensive planning process for an SBHC; and (3) Establishing a sustainable SBHC model that meets identified community needs.

Cardi-OH ECHO Quality Improvement Cardiovascular Series

Cardi-OH is recruiting Ohio primary care practices to participate in an innovative quality improvement (QI) initiative using the Project ECHO model.

HOW IT WORKS

• Practices design, implement, and evaluate a QI project that addresses an important problem in cardiovascular disease prevention or management.

• Monthly 1-hour virtual ECHO clinics featuring didactic presentations led by subject matter experts, followed by a discussion of practices’ progress in QI efforts.

• Monthly data submission followed by a 1-hour QI coaching session to review your practice’s individual QI project.
